Anyone requesting a helmet MUST come in person to Key West, FL to be properly fitted. Helmets cannot be mailed as required by the grant.

Please fill out the following online form to request a free bike helmet. Someone from the Florida Department of Health in Monroe County will call you within a week to schedule an appointment for your bike helmet fitting. Appointments are currently available only at our Key West location.
