Please fill out the following online form to request a free bike helmet.  Someone from the Florida Department of Health in Monroe County will call you within a week to schedule an appointment for your bike helmet fitting.  Appointments are currently available only in our Key West location at 1100 Simonton Street in The Gato Building.
