DOH-Monroe Well Being Survey Underway

Monroe County, Fla. – Every five years, the Florida Department of Health in Monroe County (DOH-Monroe) renews its Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP). 

The Well Being Survey is a critical part of this process and is available to all residents of Monroe County (age 18+) to provide their input on health issues they would like to see addressed. 

Information gathered from the Well Being survey will help identify the top community health priorities facing Monroe County and help DOH-Monroe to generate appropriate goals and objectives to improve these priorities over the next five years.

All residents (age 18+) are invited to take the survey.
