"Vaccines To You" Employer Registration Form

 Get Vaccinated!

 

The Florida Department of Health in Monroe County (DOH-Monroe) will provide free vaccines and administration directly to any employer or association in Monroe County through it's "Vaccines To You" program.  DOH-Monroe nurses wiil bring the vaccines and all necessary medical supplies to administer the vaccines to your employees or members at your place of business at a convenient pre-scheduled time for your business. 

Available vaccines include:  influenza (Flu), TDAP, hepatitis, HPV, shingles, pneumoccal, rabies and MPOX. (You may select multiple vaccines if desired)

Please complete the "Vaccines To You" registration from below and you will get a phone call within 1 week from DOH-Monroe to discuss the details and schedule your immunization event.

Under Florida law, e-mail addresses are public records. If you do not want your e-mail address released in response to a public records request, do not send electronic mail to this entity. Instead contact these offices by phone or in writing.
