The Florida Department of Health in Monroe County can distribute Death certificates issued in Florida from 2009 to current date.
Death certificates WITH cause of death is limited to the decedent’s family members, guardians and beneficiaries. Documentation is needed.
Death certificates WITHOUT cause of death are distributed to anyone 18 and over with valid identification.
Cost is $13 per copy. Payment can be made with cash, check or money order either in person or by mail. Payment can be made with a credit card in person, but not by mail.
Anyone requesting a copy of a death certificate must complete an application and present a valid form of ID, such as a driver's license or passport.
