Florida Department of Health in Monroe County

The Florida Department of Health in Monroe County can copy and distribute any birth certificate issued in Florida as far back as 1930.

Birth certificates are not public record: Access is limited to individuals 18 years of age or older who would like a copy of their own birth certificate and to parents or guardians (who present court documents verifying legal custody) who would like copies of their children's birth certificates.

Cost is $20 per copy. Payment can be made with cash, check or money order either in person or by mail. 

Anyone requesting a copy of a birth certificate must complete an application and present a valid form of ID, such as a driver's license or passport. 

Hours are 9 a.m. to 4 p.m. weekdays. The office can be reached at 305-676-3840

Alternatively, birth records can be obtained at the the local Tax Collector's Office in Key West and Key Largo.
