Monroe County Community Health Survey - STOCK ISLAND

WE NEED MORE STOCK ISLAND TEENAGERS TO TAKE THIS SURVEY!  TEENAGERS AGE 14+ WHO LIVE OR WORK ON STOCK ISLAND AND TAKE THE SURVEY BETWEEN THURSDAY, SEPT. 15 AND TUESDAY, SEPT. 20 WILL GET DOUBLE THE REWARD!  TEXT THE SURVEY LINK TO YOUR FRIENDS IF THEY ARE 14+ AND LIVE OR WORK ON STOCK ISLAND!

The Florida Department of Health in Monroe County (DOH-Monroe) is conducting a community health survey for residents and employees of Stock Island.  The survey will help DOH-Monroe better understand the needs of the Stock Island community as it relates to healthy living.

DOH-Monroe has set a goal for 700 completed surveys.  The survey is only 2 pages long and takes about 5-10 minutes to complete.  Answers will be kept anonymous.  The survey is available in English, Spanish and Haitian Creole with the links below.

Thank you to anyone who lives and/or works on Stock Island for taking the time to complete the survey!

Anyone who completes the survey has the option to include their name and address at the end of the survey to receive a $5 food gift card in the mail from the FDOH as a reward for survey participation.  This is completely optional and these fields can be left blank if preferred by the person taking the survey.  All $5 food gift cards will be mailed out by the FDOH in August of this year (2022).
